GROUP BOOKING - Double Dive: ex-HMAS Brisbane wreck

This is a CUSTOM BOOKING FORM for DIVE SHOPS.

The final prices and FOC's are determined on the day prior to the trip departing. Your company will be invoiced for the total amount. We are not collecting any money through this booking process.

The prices on this booking form are set at $161 + hire gear (tanks, nitrox, etc). Please contact us if you have questions regarding final prices and FOCs.

Please select the TOTAL people coming on the trip (including the tour leader/instructor).

You will be required to enter credit card details to make the booking, however it will NOT be charged at the time of booking. Please specify in the special requirements section if you require the card charged the day prior to your trip OR if you would prefer to be invoiced.

Enter as many of the following details for each participant prior to booking:

• Name
• Cert level
• # Logged dives
• Date of last dive
• Body weight in kg
• Diving air/nitrox

By filling in this form you will reserve the spots directly without contacting Scuba World. As a small business, we appreciate any efficiencies we can ascertain easily without inconveniencing you.
